The Philippine Embassy in Bangkok has released a new set of forms for its constituents wishing for application or renewal. Three new formats are now out for 3 group classifications.
White form: For renewal of ePassports for adults.
Pink form: For all minors (below 18 years old); for new or renewal of their ePassports.
Yellow form: For lost or damaged passports, changing to married surname, renewal of passports that are not ePassports; and/or all other cases.
These forms are readily available for download online through the DFA’s website.
